Key Responsibilities

  • Direct and oversee home health services delivery, ensuring diagnosis-specific patient care
  • Manage patient care services and personnel, including assignments, referrals, assessments, and care plan updates
  • Coordinate services with providers, vendors, and facilities
  • Ensure timely initiation of patient care per regulatory guidelines
  • Oversee physician orders, staff assignments, and team communication
  • Review and approve care plans and integrate physician orders

Qualifications

  • Education: Associate’s Degree in Nursing (required)
  • Experience:
    • Minimum 2 years as a Registered Nurse in home health or behavioral health
    • Minimum 1 year in a supervisory role
  • Licensure: Active RN license in the state
  • Technical Skills:
    • Familiarity with enterprise applications (e.g., Workday, HomeCare HomeBase)
    • Proficiency in Microsoft Office (Word, Excel, Outlook)
